We’re looking for someone whose life could be transformed by a new smile.

3. Additional photo of your choice

Must be 21 years of age or older

Must reside in the Tampa Bay area

Must be available to attend all required appointments

Must be dentally and medically eligible for treatment following a comprehensive examination

Must be willing to participate in before, during, and after treatment photography and video

Must agree to the campaign’s media and publicity terms

Must be able to complete the required treatment within the campaign timeline
